Stay at home if you have coronavirus symptoms

Stay at home if you or someone in your household have Coronavirus (COVID-19) symptoms which are either:

• a high temperature (above 37.8 degrees)
• a new continuous cough

Do not go to a GP surgery, pharmacy or hospital.
You do not need to contact 111 to tell them you’re staying at home.
You can complete a self certificate for absence so you do not need to contact us for a doctors medical certificate (or ‘sick note’)

Home visits are only available to patients registered with Church Lane Surgery. Patients must be housebound because of illness or disability.

If possible, please call the centre before 10am if you require a home visit on: 01376 552 474

A doctor or nurse may call you back to see if your problem can be dealt with over the telephone. Depending on the situation, it may be appropriate to send a nurse or arrange a hospital attendance for you.

Visiting is extremely time consuming so please try to attend the centre if possible.
